Nominate a family physician for Oregon's Family Doctor of the Year

Each year the OAFP chooses from among its members an Oregon Family Doctor of the Year. The award is announced at the group's Annual Scientific Assembly in May. We try to keep the name of the winner a secret and surprise them with the announcement. Those who win are usually very surprised to see the amount of admiration and support they receive from their communities.

Doctors may be nominated by their colleagues, communities, or families. If you have someone you would like to nominate, please print several copies of THIS FORM. You will have a stronger nomination if you have other people, in addition to yourself, fill out the form. Suggestions of people to ask include, people the physician works with in the office or clinic, other physicians in the community, patients, civic leaders, or people related to the physician's outside interests (such as church, scouts, sports, charity projects, etc).

Please send the forms to our office by March 14, 2008.
